Coordinated care in Primary Health Care in Jedlnia-Letnisko

396 patients have been enrolled so far in the coordinated care program at the Independent Public Healthcare Facility in Jedlnia-Letnisko. The service has been operational since October 1, 2022. Coordinated care is provided to patients with chronic conditions treated in primary healthcare facilities. Within these activities, the primary care physician collaborates with specialist doctors, primary healthcare nurses, and dietitians. They focus on prevention, diagnosis, treatment, and patient education in the fields of cardiology, diabetology, endocrinology, pulmonology, and nephrology.
Piotr Kowalski, the Director of the Healthcare Center in Jedlnia-Letnisko, announced that the facility offers additional examinations and consultations with doctors in two specialties: cardiology (since June 2022) and endocrinology (starting from October this year).
– So far, we have enrolled 395 patients in the coordinated care program. Within the cardiology activities, we have provided 259 consultations, 580 in the field of dietetics, and 754 educational consultations. These consultations are conducted by doctors or nurses. We have provided 23 endocrinology consultations – said Director Piotr Kowalski.
As part of the coordinated care program, specialist doctors conduct in-depth interviews, examine the patient, analyze the results of our diagnostic tests, evaluate the current state of health, and establish an individual action plan with the patient. They also provide the patient with the plan for implementation.
